I code for HCC Risk Adjustment. CMS 6.4.1 "physicians should code all documented conditions that co-exist at time of encounter/visit...management.
I am finding chronic only in the PMH and not addressed in the office note. Wouldn't it require something to tie in that the chronic was addressed in this visit in order to code it?

Hello, yes, I code for HCC and my physicians address the chronic conditions when he assessing the condition and he will note how he is treating the condition and if it's stable , he will address if it's chronic and that will help for your audits.

I also audit for HCC Coding and per HCC Risk Adj guidelines you can not pull Dx codes from the PMH, all chronic conditions have to be documented and addressed under the assessment and plan of the chart note in order to be reported, e.g. Diabetes - controlled.
