If it were me coding it, the choice would depend on any other presenting diagnoses. If they have either of the gastrointestinal conditions listed in the primary description of K52.9 (gastroenteritis or colitis) I'd use that code. If it's unexplained or un-sourced diarrhea, I'd use r19.7.
