I am a new IP coder; I tried looking the answers up in the ICD 10 diagnosis code book but I cannot find it . when the H & P mentions the chronic diseases and tobacco use/exposure we code it for that day but what about the subsequent days when they are not mentioned?

Are they coded every day? or ONLY when documented? I read somewhere during my google searches that for the tobacco exposure and/or use it is coded ONLY when applicable which i take to mean only when documented ... but i cannot find a solid resource . I would appreciate any and all assistance.
