I need more clarification from renal failure vs CKD
This patient has a problem list of CKD stage 2
but the doctors notes only state chronic renal failure.

This patient also has hypertension.
Should code CKD go with the hypertension and list the renal failure separate?
Please advise as to codes.

Chronic renal failure in ICD-10 is coded as N18.9 which is in the same category as CKD and since CKD stage 2, N18.2, is more specific, you would not code the chronic renal failure separately.

If the patient has hypertension and the provider has not documented that they are unrelated, then you would code from the hypertension with CKD codes I12 or I13, as appropriate, per the guidelines.
