The guidelines state that CKD and HTN are a presumed relationship. Does the provider need to first document and agree that the HTN is due to CKD before the HTN code is changed to reflect the relationship?

The relationship is reversed. Hypertension increases the risk for kidney disease. The CKD is presumed to be caused by the HTN. (I12.-)

Specific documentation is required for secondary HTN (I15.-)

CKD is presumed to be due to HTN. Use code I12.0 or I12.9 and then code the level of CKD N18.-
The provider does not need to document a linkage. We assume a link
