My doctor saw a patient for back pain and headache. Under medical history in patient record : Doctor listed patient has hypertension, hyperlipidemia, diabetes, depression, GERD. PSA.
On the claim form, I coded back pain, headache. But risk adjustment auditor after reviewed medical records, they coded back pain, headache, hypertension, hyperlipidemia, diabetes, depression, GERD, PSA and sent us an adjusted claim with all dx and request signature if we agree with all codes. We have this patient with 3 different date of services and auditor coded all same diagnosis codes under medical history on each claim form. I am a new coder and I don't know what should I do. Do I agree with them or not, please help.
