Clarification on Billing 64450 Units for Medicare and Medicare Advantage Plans

How many units of CPT 64450 can we bill per visit for Medicare and Medicare Advantage plans?

Also, if a patient receives 64450 injections once every 3 days and we bill 16 units for each date of service, will the insurance reimburse for all units?

For example, if a patient receives a 64450 injection today and we bill 16 units, can we schedule another 64450 injection on 10/16/25 and bill that visit with 16 units as well?
