Wiki Clarification on billing a nurse visit for a PPD test reading

Our practice wants to start billing CPT 99211 for the reading of a PPD test result by a nurse.

My question is, if the initial test was administered during an office visit where the patient was seen by a physician, is the E/M work done by the nurse for the test reading bundled into the initial office visit, or would 99211 be charged for the reading since it is occurring on a different date?
