Clarification on code 404.10

How do you code for 404.10? What medical criteria would the patient need to be in to support this code?

Hypertension without heart failure with chronic kidney disease stage 1-4...

Would the patient have to have some sort of cardiac history to be able to bill this code? Old MI? Etc.?

My physician is billing for this code and although the patient is CKD 3 with hypertension, there is no indication in the note to support any type of heart failure, or a history of heart problems..

Well, that's really up to the doc to decide...but the code description is without heart failure, so I would think there doesn't need to be any documentation...unless I'm misunderstanding your question... :confused:

Hi,

It was just perplexing to me why the physician did not use 403.10 instead.. Hypertensive CKD 1-4 Benign.

I guess what was throwing me off was the fact that he selected the HTN w/out heart failure CKD 1-4 with no current or past history of heart failure....
