A patient came in and had a cleft palate repair with bone graft, fistula repair, close nasal floor and a bone marrow harvest. The CDR for the CPT code for the cleft palate repair, 42210, talks about how the "physician harvests bone from the hip or skull and closes the surgically created wound." I know guidelines from the AAPC website state not to code the bone harvest as it would be included in the 42210 but would we code 38220 for the Bone Marrow Aspiration? I am confused because this is not the same thing as "harvesting bone" however the intent of the Bone Marrow Aspiration was to mix the bone marrow with an "infuse large kit" to fill in the defect.
