Humana Gold is starting to deny 87430 with modifier "QW" as "CLIA services billed are above PPM certification." This "swab" of the throat for a quick diagnosis of strep is done in the office, before prescribing antibiotics. Should we be using 86403 (strep quick test)?

The last place I worked used 87880 for the rapid strep swab done in the office.
