SO MY DOCTOR INSIST SINCE 87880 AND 82274 AR CLIA WAIVED TESTS THAT WE SHOULD NOT HAVE TO RENEW OUR CLIA CERTIFICATES AS LONG AS WE USE THE QW MODIFIER WHEN BILLING. I HAVE BEEN USUING QW AND STILL GETTING DENIALS. ANY IDEA HOW I CAN REMEDY THIS?
 
If you are doing ANY testing, you still need to have a CLIA certificate. There are several types of CLIA certificates. If you are performing only waived tests, you need the CLIA Certificate of Waiver. It's form CMS116 and gets mailed to your state agency. The CMS link with information and the form is below:
