We have a current patient on Accutane with UHC and they keep denying her HCG stating that our CLIA certificate level is PPMP and the codes certificate level is "waiver.". We were advised to have our certificate level changed to "Waiver". I just don't think this is correct, our CLIA certificate has always been the same and I'm not having this issue with any other claim or insurance policy. Any one familiar with this?

Have you used modifier QW? I know for UHC for we have to use the modifier QW for all our CLIA waved tests.
