Provider documents a procedure done in the office setting but also documents to no charge the procedure. I am sure it's due to the cost. As a coder and biller, do i charge it or not charge it?
 
If the provider has said it is no charge, then I would certainly not charge it without talking to your employer first. There may be a valid reason that the provider (or patient) doesn't want this charged or billed. But there are compliance risks involved with giving away free services to patients, so this is something that needs to be looked at by a manager or compliance specialist to make sure that the provider is doing the right thing in this case. Your employer needs to give you guidelines on how to handle situations like this.
 
Agree with @thomas7331. A practice is permitted to occasionally use discretion in not charging a procedure. However, this falls under compliance and regulations rather than coding guidelines.
