To code CPT for Closed fracture, What should be the documentation?

Site of fracture (distal radius or shaft of radius). Type of fracture (oblique, transverse, comminuted). If the fracture is intra-articular that should be documented. In coding you assume that the bone is displaced. If it's not displaced, that should be documented.

If your provider is going to bill for fracture care, that should be documented (not assumed). The physician should indicate in the assessment that they are initiating fracture care.
