If a patient receives a closed trimalleolar reduction (with traction) in the office and is scheduled to have sx four days later, do you bill the reduction with the e/m or just the e/m and wait to bill any fx treatment until the sx is billed?

Any help/suggestions is greatly appreciated! :eek:

I would bill out the 27818 for the closed reduction, allow it to get the 90 day Global starting that day, then on the Sx CPT add modifier -58 to represent the "more extensive" procedure. Modifier -58 doesn't re-start the Global date and it allows the more extensive procedure to get paid in full without reduction.
