Can anyone tell me what the PICA means at the top of the form
 
Page two will give detailed information for each box. PICA is not a required field, think it is for patient insurance claim?
 
PICA meaning

I think PICA is a unit of type size and line length, equals to 4.2 mm. Every information on CMS-1500 gets updated in the specified size. In an inch vertically 6 lines gets created or can be created and horizontally 10 characters are allowed to enter or 10 characters gets entered. CMS-1500 form size is 8.5 Inches horizontally and 11 inches vertically.:)
