I just quickly went through the 89 page CMS E/M guidelines manual and they did not publish (that I could find) the change in the 1995 documentation guidleines that states you can use the status of 3 chronic or resolved conditions as an extended HPI. In September of 2013 it was decided the 1997 HPI guidelines can be used with a 1995 exam. I would like to see it in official published form, can anyone pint me in the right direction?
