Wiki CMS Letters asking for "Invoice"

We are a independent clinical laboratory. Recently we started receiving letters from CMS stating that they need an invoice before paying claims for codes 81350, 81400, and 81401. These are laboratory tests, not something that we bought and paid for. There is no invoice. We have both called and written letters to Cahaba and CMS, but they do not seem to understand or even care. Right now we have 1300 of these letters, holding up over a million dollars in claims, and we can find no resolution. Is anyone having this problem, or does anyone have any suggestions on what to try next? For the record, these codes always paid with no issue until recently. Thank you
