CPT does not list an add-on code for 15040. It would appear if the harvest of skin is greater than 100 sq cm you would code 15040 with multiple units to indicate a second or third 100 sq cm over the first 100 sq cm.

The procedure was harvest of split-thickness skin graft for subsequent use, so i have to use code 15040.... the next visit was the application of the split-thickness graft, 15150 was the right code for the visit?
