The physician numbs the right lateral abdominal wall with local anesthetic. The physician places a large needle through the skin into the liver, maneuvering the needle through the liver into the intrahepatic portal vein, injecting contrast under fluoroscope for localization. The physician places a wire through the needle, retrograde into the portal vein. The physician removes the wire over the needle, and advances a catheter over the wire, through the skin and liver into the portal vein. The physician secures the catheter with suture. Alternative methods include placing a catheter via the internal jugular vein or using a transsplenic approach.
