can this code only be used for open procedure? If Dr. does exploratory lap and repairs bladder with sutures (done during previous surgery, different doc) what code should I use?

Please check whether this could help:
49000 for Exploratory laparotomy with or without biopsy ; or 49002 for reopening of recent laparotomy.
Suture of the bladder does not have to be reported separately( it becomes a part of it). Physician's documentation and diagnosis code would support
