Hi all, quick question. When the provider documents "fracture fragment" on a radiology report (e.g., "redemonstration of the distal ulnar fracture fragment"), do you code for a fracture of the bone they mention, or do you just code for a loose body in the joint? I assume if there's a fracture fragment that means the bone was indeed fractured, so I have been coding for the full fracture, but I just wondered what others do in this scenario and if I'm doing it correctly. Thank you!

This means a loose body in ulnar region so I d add code dx M24.031 or M24.032 depending which side right or left. I hope this helps
