Code for Incision and Removal of Foreign Body, Intradermal; Simple (Not Subcutaneous)?

Hi fellow AAPC members,

I am trying to assign the appropriate code for the incision and removal of a foreign body (a lip piercing in which the skin has grown over the piercing on the inside of the mouth). The procedure note documentation indicates that the depth was intradermal.

Based upon the 10120 CPT code description, the incision and removal must reach the depth of the subcutaneous layer and therefore, 10120 is not valid for the dermal layer. Does anyone else have any recommendations for the incision and foreign body removal from the dermis?

My understanding is such that the procedure will be included in the E/M code since there doesn't appear to be a valid code for this procedure involving the dermal layer. Am I mistaken?
