I code for a Family Practice. Say our physician refers a patient for a routine mammogram (Z12.31), do we include this code on our billing as well just because a referral was issued?
 
You would code the reason the patient came in. If they come in for their physical, It's one of the Z00.- codes. If they came in because they were sick, it would be what the findings are. The fact they were referred for a mammo shouldn't affect DX selection for the encounter. You can add the Z12.31 to the order for the mammo so the lab knows how to bill but it doesn't need to go on the claim.
