This is what I found in my quick research of "end stage heart failure":

"Patients with end stage heart failure fall into stage D of the ABCD classification of the American College of Cardiology (ACC)/American Heart Association (AHA), and class III–IV of the New York Heart Association (NYHA) functional classification;"

Per ICD9Data.com and came up with 428.0. Look at all the descriptions of that code and you will see reference to those stages.

I would reasearch a bit more and maybe query the doctor and past medical history to clarify if it is congestive heart failure. That's my opinion!
