I am looking at an op report that states...

Supracervical hysterectomy
Abdominal sacral colpopexy with Pelvitex advantage tension free suburethral sling
Posterior repair with repliform graft.

The findings were first degree cystocele, first degree uterine prolapse, and third degree rectocle.

I am looking at codes 58180, 57280, 57288, and either 57250 or 45560.

Any thoughts??
