Assume the following is all you have. How would you code this?

Site # 1: Benign nevus (L breast x 1)
Procedure: Destruction of benign lesions with shaving.

- Benign nevus (L breast) Procedure today: Destruction of lesions with shaving.

This is what I come up with:

Benign Nevus of the breast:

Dx: 216.5 (Benign neoplasm, skin, breast)
CPT: 11300-11303 _depends on the size (Shaving of a single lestion, trunk)

Skin tags:
DX 701.9 (Unspecified hypertrophic and atrophic conditions of the skin)
CPT: 112.00 (Skin tag removal- up to 15 lesions)

Yes it does.

I'm doing an audit of our charts. Because the coding happens in the rooms about 90% of the time, I've just had a problem fall into my lap and I need to figure out how big it is.

I ask here because I do not yet trust myself since I've not even had my CPC-D for a year.
