I am trying to find out if one of our medicare patients see an outside doctor and that doctor has ordered lab, do we have to use the ordering doctor's name on the claim. For whatever reason, the patient returns to our office for this lab and we send it out with the pcp's name on it not the ordering doctor.
 
You cannot sign an order for another physician, that would be fraudulent activity. If the primary care provider is the one ordering this lab, they should be going to their PCP for results of the lab. If they come to you to redo or do another round of the lab then it would fall under the ordering provider.
