what diagnosis would be recommended to use if a Surgery did not occurred or started due to equipment failure.

I would use the reason for the surgery (the patient's actual diagnosis) as the primary code, and add Z53.8 - Procedure and treatment not carried out for other reasons - as a secondary code.
