Can anyone provide information on how plans pay codes with a $0.00 rate?

The services are covered through Medi-Cal, why is there a $0 rate for some procedure codes? Would these be considered as an encounter?
 
Well, it looks like that means one of two things: either they don't cover unlisted molecular pathology, or it's priced individually based on what was done. I would call the Medi-Cal Provider Helpline and ask.
