The procedure is repair 1st metatarsal fracture non-union, repair 5th metatarsal fracture non-union, repair 5th metatarsal fracture mal-union. How to code this?

I'm not an ORTHO expert. You may get a more qualified response by posting your question in the Orthopaedics forum.

However ...

Your question mentions two distinct bones (you have 5th metatarsal listed twice). I believe you can only code once per bone.
