There are multiple different mechanisms. The most common are code-over-code and chart-over-chart. Of course, some organizations cook up their own to meet specific needs and might vary greatly from those mentioned.
Chart-over-chart is very general and looks at each encounter as a whole, without any specifics being taken. It is also--in my opinion--one of the most troublesome methods. It's a form of auditing laziness.
Code-over-code, on the other hand, would provide equal weights to various codes assigned to the records. I've seen modifiers both determines as their own category and also excluded. This is the best measure of true coding accuracy and provides a very granular look at how a coder is performing. It may also demonstrate where a coder is failing or needs development.
