Does anyone know the appropriate way to code "cerebrovascular disease"? The documentation simply states cerebrovascular disease and the physicians typically choose code 436, 437.1 or 437.9. By the descriptions, I would personally code 437.9, but if a late effect such as hemiplegia is also documented, would you code both? Guidelines say you can if there is a late effect from an old cva, and the patient has a current cerebrovascular accident? I don't understand that, since the 437.9 code doesn't describe as a cerebrovascular accident.

In the past, I would normally only code hx of cva (V12.54) or if a late effect was present, use codes from 438 category. I should add- I've always worked in homecare/hospice not having to use the acute cva codes. Any suggestions/info would be greatly appreciated. Thank you.
