Can anyone help with this...when a patient is seeing a provider for a previous episode of chest pain but has no other symptoms in the HPI or exam what would the diagnoses should be considered?

I disagree. If the patient has no chest pain at this encounter then we cannot code it. The documentation will not support this code if the patient is asymptomatic. If this is a followup then use a V67.x for follow up.

I agree with Debra. Since there was no existing symptoms at the time of show, Certainly I would go with the V67 series if it is a follow up. Or else I would go for v71.9 / v71.89.
