If you have a patient that comes in for colonoscopy with history of colon polyps but no symptoms, is it appropriate to bill 00182 with Z12.11 and Z86.010 or should you only use Z86.010

1. screening code
2. hx of colonic polyps/family/personal hx of intestinal cancer.

Correction:
have to check order if the physician indicated it was for screening and when it was the last colonoscopy. additional dx if any polyp was removed from this current colonoscopy.

Z86.010 may not be a primary code. If no polyps are found, use Z12.11, Z86.010 in that order.
