If an inpatient has a decubitus ulcer treated, but no stage is documented - just a description of the ulcer (open down to bone). Patient had a BKA.

Would you code the stage by the description - or would you always have to query?

I could not find clear direction in the coding guidelines on this.

As per the guideline note in the code 707.00 (decubitus ulcer), whenever using this code series you have to use additional code to specify the stage of ulcer (707.20).

Hello, code 707.24 pressure ulcer stage IV:
pressure ulcer with necrosis of soft tissue through to underlying muscle, tendon, or bone, you have to add the code from subcategory 707.0 to identify the site of the pressure ulcer, or quire the doctor::
 
patient with a rare condition called cilium incarnatum externum which is a condition in which there is a lash misdirection into the subcutaneous space of the eyelid, and it is not trichiasis which is a
completely different condition, for this patient I had to make an incision in the upper eyelid’s skin, expose the misdirected lash and remove it. And that was done in the minor surgery room.
What would be the proper ICD code for this condition and the proper CPT code for the procedure
