If the provider has documented all three conditions, Diabetes Type 2, HTN and CKD stage 3a would you use I12.9 AND E11.22? I wasn't sure if I could code those both for the CKD. Thanks

I have a similar situation; however, the documentation indicates type 2 diabetes mellitus without complications. Would you still code E11.22, or E11.9?Yes, you'd code I12.9, E11.22, and also N18.31 for the CKD 3a.
(Assuming that the provider didn't specifically state that the CKD was unrelated to either the hypertension or diabetes. The "with CKD" linkage is assumed to be related, unless the provider specifically documents that it isn't.)
