I am preparing myself for the CPC exam and I have the CPC official study guide but not the ICD-9 CM for 2012. I have some doubs about how to code a none confirmed or suspected diagnosis(outpatient setting). I saw a post in a forum where somebody explained that according the new guideliness is ok to code an unconfirmed diagnosis as diagnosis, but according to the official study guide of 2012 says we dont code for a diagnosis untill its certain and code the symptoms intead.

You cannot code suspected, probable, or possible, diagnosis. Except for inpatient facility coding physician and outpatient cannot code these conditions, you will need to code symptoms.
