Can anyone help me find the coding guideline for coding of diagnostics (xrays; cardiac echos; carotid ultrasound; mri; ct scans etc.)? The other coders in the office just code OPD charts without any of the findings. They feel this is not important and said they just go back and look at the xrays when the billing department advises that the file will not pass the billing scrubber.
I hold files until the results are back and evaluated/signed by a physician. If finds are positive and stated as such/signed by physician then I put this as the primary diagnosis and the symptoms second.
Any idea where in the coding guidelines I can find this information?
