Wiki Coding Elevated Glucose for Diabetic Patient

I have a patient who his diagnosed as diabetic. He came in for a follow up and on the assessment portion of the note, my provider has elevated glucose listed as the diagnosis on his assessment instead of diabetes. Since the fluctuation of blood sugars is part of the disease process, should this be coded in lieu of diabetes or should he have instead documented diabetes controlled or uncontrolled and leave of the elevated glucose?
