How can you get 69210 paid when billed with office visit and vaccines? It keeps getting denied as inclusive..We have used the 25 mod as well as a 59 and it is still getting denied...the doc actually uses the curette to remove the wax...

What insurance? Our medicaid carrier will not pay for 69210 when billed with an office visit. They have even issued a written policy.
