If someone comes in and they have dermatitis, so we give them a shot due to the dermatitis causing pruritis (itching), would I code the pruritis as the reason for the shot, or the dermatitis? I do the dermatitis because it is the definitive diagnosis, but I am not sure if I should use a symptom code for the link instead.
 
Thank you. What about assumptive diagnoses?

Say a patient comes in for itching and redness and the doctor diagnoses them with T captits, and never does a test to confirm the TCap. Is it a definitive diagnosis that I can use?
