When coding for casting in a doctor's office setting, (POS 11) is it appropriate to also add the supply charges? If so, should we us the HCPCS Q-codes e.g. Q4040, Q4012? or code 99070 ? Or would the supplies be bundled into the E/M or the procedure code for the cast application?

Medicare will, in general, pay for the casting supply as well as the application code, this is usually a Q code. Commercial plans, however, often want an A code for cast supplies.
