I have a question, im billing 11046 and 11043 to medicare and they are saying that dx682.2 is not medically necessary. Where can I get a list of these codes. Please help!

Check with your MAC for LCD 24374 - Treatment of Ulcers & Symptomatic Hyperkeratoses.
