I work for a cardiology office and we are receiving denials on services for which an e/m visit and pacemaker or defibrillator are done on the same day and at the same time. We are billing for both example: 99213 and 93280. We were told as of the new pacemaker and defibrillator codes went into effect there was no modifier needed with the e/m code. Should we be appending the 25 modifiers on the e/m codes or not with these situations. We are still seeing these denials from various payors.

Is the 93280 a status "S" or "T"? If so, then I would append the 25 modifier. JMO tho, I don't code a lot of cardiology at all.
