Hello,
do anyone know how to code for an insertion of an IUD when a recent IUD had fallen out (about two weeks later)?
Is there a modifier to let the payor know this is due to complication?

HELP!
 
We utilize the modifier 76, since none of the other modifiers apply (such as 58 as there are no global days). We submit with medical records and a letter explaining the reason for the additional service.
