My primary care office does not have RNs or LPNs so PPD visits are most often done by our mid-level providers, and occasionally our doctors. We have gone back and forth for years on the best coding practices for PPD placements and reads. Also, most of the time the placement is done in the context of a Wellness or a form completion appointment so we are charging for that visit and then the supply at the time of administration. The read is the tricky part for me...is it included in that first visit that we already charged out, or should it be a 99211, or even a 99212 since it's an APRN or MD? Any input is appreciated!!
 
The level of the visit should not be upcoded just because the service is done by an MD.

The return read is 99211 if negative. If positive and they are seen right away, it would become whatever E&M service is performed.
