Medicare patient comes in to office receive two different shots for two different drugs.
Do they bill just for one office visit and 2 administration fees or just the administration fees plus the drug charge??
 
if the reason for the visit is to receive the injections then you code the admin and drugs only. If the visit was not for injections but they were decided as a part of the plan after examination, as long as the documentation can support the 25 modifier then you may code the ov with the 25 and the admin and drug codes.
 
yes it would be for ER as well even the facility if it is planned ahead of time as an injection it would be injection only and a separate administration charge for each injection using a 59 modifier for the second and subsequent.
