Wiki Coding from the consent to operate form

Our anesthesiologist performs 62311 as an outpatient procedure. He never lists the diagnosis on the procedure note. It was stated that the diagnosis can be coded from the consent to operate form. I would appreciate any feedback if indeed this form is a valid piece of documentation that can be used to code from. Thank you!
